To book a school Jazzistry program

1. Open the Booking Form.
2. Save the file to your computer.
3. Complete the form, print it and email it to info@jazzistry.org or mail it to PO Box 7146, Ann Arbor MI 48107.

After receiving your booking form the Jazzistry office will contact you and discuss all the details — price, preferred length of performance, size of audience, ties to curriculum or other school themes, performance space, payment, etc.

We will send you an agreement drawn up from the information on your booking form and our follow up conversation.

Your signed agreement is needed to officially reserve your date. Once we have your signed Agreement, you are guaranteed the time and the day indicated.

A useful preparation checklist comes with the agreement. Completing the checklist will insure a smooth performance. Please contact us with any questions or concerns about the checklist.
